The problem it addresses is structural. Healthcare interoperability standards exist (HL7 v2 and FHIR among them) but a standard permits local variation, and hospitals use it. Two sites running the same EHR can present the same clinical fact differently, so an integration is rarely finished when the specification is met. The difficulty is less the API surface than accumulated knowledge of how these systems behave in practice.
