What it does
Organisations configure a monitoring programme (which readings are collected, how often, what triggers an alert) and patients use the resulting application. Readings from connected devices and patient-reported symptoms arrive in one clinical view.
Because the same platform serves a hospital-at-home programme and a decentralised clinical trial, the interesting constraint is configurability under regulation: the thing being configured is a regulated product, so the configuration surface cannot be open-ended.

Escalating to a person when a reading crosses a threshold
A remotely collected measurement or answer (a vital sign, a symptom score, a missed check-in) passes a limit set for that patient, and the product raises it with a named person: a monitoring clinician, a care team, or the patient themselves with an instruction to act. The pattern is the threshold, the alert, and the route by which a person becomes responsible for what happens next.
